We are seeking a dedicated and compassionate Care Coordinator to join our team. Duties
- Act as the primary point of contact for clients, families, and care providers, ensuring clear and compassionate communication.
- Develop personalised care plans tailored to individual needs, collaborating with healthcare professionals and service users.
- Schedule appointments, coordinate services, and monitor ongoing care requirements to ensure consistency and quality of support.
- Maintain accurate records of client interactions, care plans, and service delivery using IT systems.
- Provide guidance and support to clients and their families, helping them navigate available services and resources.
- Liaise with external agencies to facilitate smooth delivery of care services.
- Ensure compliance with organisational policies, safeguarding procedures, and relevant legislation.
Experience
- Proven experience in a caregiving or healthcare coordination role is desirable.
- Excellent communication skills in English, with the ability to listen actively and convey information clearly.
- Proficiency in IT systems, including data entry, scheduling software, and basic troubleshooting.
- Experience in teaching or training others is advantageous but not essential.
- Prior experience working with diverse populations or vulnerable groups will be considered an asset.
