Overview
We are seeking a dedicated and compassionate Health Link Worker specialising in adult autism to join our team. This role involves providing tailored support and health education to adults with autism, promoting their well-being and integration within the community.
The successful candidate will work closely with individuals and healthcare professionals to deliver person-centred care and guidance. This position offers an opportunity to make a meaningful difference in the lives of autistic adults, fostering independence and improved quality of life.
Responsibilities
- Support autistic adults to access aspects of healthcare within primary care
- Assist autistic adults to make and attend healthcare appointments including annual health checks
- Identify and respond to requests for reasonable adjustments
- Support clients to understand outcomes and results of appointments and checks
- Build relationships with clients by introducing them to the service offerings AIM can provide
- Update clients’ ‘What matters to you’ plans and DIALOG records
- Create health passports and alert cards
- Build relationships with GP Surgeries, CMHTs, CTTs and other health providers ensuring they are aware of what AIM can provide
- Record details of client interaction in our internal Charity Log portal
Skills
- Knowledge of Autism in Adults whether in work, volunteering or lived experience surroundings
- Understanding of public health practices and medical terminology relevant to adult autism support
- Excellent communication skills, and the ability to convey complex information clearly
- Empathy, patience, and a person-centred approach when working with adults on the autism spectrum
- Organised with the ability to manage multiple cases simultaneously while maintaining attention to detail
- Ability to collaborate effectively within multidisciplinary teams and engage confidently with clients from diverse backgrounds. This role is ideal for individuals passionate about improving health outcomes for adults with autism through education, advocacy, and personalised support.
