This is a 3-month contract role with a local authority, aimed at providing high-quality advice, guidance, advocacy, and practical support to young people aged 17½ to 25 who are leaving care. As a Personal Advisor, you will manage a caseload of care leavers, including complex and high-risk cases, ensuring they receive the necessary support to achieve positive outcomes in education, employment, training, accommodation, health, finances, and independent living.

Manage an allocated caseload of care leavers aged 17½ to 25, including vulnerable, complex, and high-risk cases.

Build positive and effective relationships with young people to engage them in planning for their future and achieving their goals.

Undertake assessments of need, risk, strengths, and aspirations to inform support planning and interventions.

Develop, implement, monitor, and review Pathway Plans in partnership with young people and relevant agencies.

Provide information, advice, guidance, and practical support on education, employment, training, accommodation, health, relationships, and independent living skills.
Experience:
- Experience working directly with young people, vulnerable adolescents, or care-experienced young adults.

Experience managing a caseload and delivering individual support plans.

Qualifications:

Level 3 qualification or above in Social Care, Youth Work, Education, Housing, Community Work, Health and Social Care, or a related discipline.

GCSEs (or equivalent) in English and Mathematics.

Enhanced DBS clearance.

Working hours: 36 hours per week.

Work pattern: 2-3 days office-based.
