Weekly hours to be arranged over a 7-day period including days, evenings, nights, sleeping nights and bank holidays, therefore flexibility is essential for the post.
You will be required to provide holiday and sickness cover.
The days and times of individual posts will be based around the needs of the clients.
The job will relocate from the Ellesmere Port area to Prescott early 2026, the ability to commute to Prescott will be vital.
Overview of Support Worker
· The role requires Support Workers with a desire to commit to a positive supportive team, focused on providing high quality support.
· Providing care and support for a young lady in her twenties who has complex disabilities.
· The client requires complete assistance with all aspects of living, including personal and intimate care, postural management, gastrostomy management and management of her Crohn’s disease.
· Delivering effective care and support is important, as is having fun and facilitating a good quality of life which is vital.
· Support workers should have good initiative and work comfortably and sensitively within a friendly, lively, family home.
· You will be part of a team of friendly and positive people, delivering a high standard of care and support 24/7 and be supported by the wider team in all aspects of your work.
· The client should be supported and facilitated by her support team when independently expressing her own thoughts, values and decisions.
· Car driver and non-smoker essential.

The post involves supporting one young adult with complex needs, which affects their whole body in such a way that they need help with all aspects of daily living, to provide support with all heath, social and personal needs.
The support worker will be required to deliver support to a high standard under the direction of and in collaboration with the client, Case Management Team, Therapy Team, Team Leader and family.
Experience in working with young adults with additional needs is preferred but positive personality, worth ethic and ability to work flexibly on your own initiatives within a family home are essential.
The support worker must have a full UK driving licence.
The client also has a pet dog. Therefore, support workers must be comfortable around pets and caring for pets.

Key Tasks
The following is an outline of the daily tasks and is not intended to be a complete list:
· To support the client in all aspects of everyday life.
· To ensure client’s privacy and dignity is always maintained.
· To act as advocate, and to be a positive role model.
· To undertake other tasks relating to the care and welfare of the client as necessary and appropriate. This will include domestic tasks around the home.
· To be sensitive to the client’s wider family, by respecting their space and boundaries.
· At times you will support the client to participate in family plans and activities, taking lead from the client and parents, while remaining mindful of the clients wishes.
· To work effectively in collaboration with other heath and social care professionals supporting the client.
· To attend mandatory training and meetings as requested by the Case Management Team, Parents and Employer.
· To always work within the guidance detailed within the Working File (Care Plans) and ensure you are always up to date and familiar with these plans.
· To work collaboratively alongside your colleagues, using a positive team approach. Work jointly to support your colleagues and communicate effectively at all times.
· To acknowledge your limitations or areas where improvements and additional training may be needed. It is essential that you communicate this quickly with your team leader and case manager, to ensure the appropriate steps are taken and support is provided.
· To maintain the client’s, well-being and safety at all times.
Main Responsibilities

- To support the clients with all aspects of daily living.
- To work in collaboration with parents, Team leader, Case Manager and the wider MDT in ensuring care and support is effective, person centred and is of a high standard.
- To follow the therapeutic guidelines and clinical plans at all times to ensure the clients heath and safety is maintained.
- To communicate with the relevant people in the event of an emergency or ill heath.
- To complete detailed and contemporaneous records.
- The Support Worker will be required to implement any therapeutic programmes and exercises as directed.
- The Support Worker will be required to make themselves available for appropriate and mandatory training as required.
- Although heath and care needs are a vital and essential part of maintaining a good quality of life, the role extends to ensuring the clients has opportunity to individual expression, social opportunities are equally as vital to a good quality of life.
- The Support Worker will become a skilled user/ facilitator of the client’s communication techniques though training and liaison with the therapy team and should facilitate and encourage the client to communicate with autonomy.
- The Support Worker will support the client in social activities both at and away from home; this may involve at times driving her adapted vehicle or using public transport.
- When supporting the client (especially in social situations) the Support Worker will maintain a positive, proactive and encouraging approach, ensuring the client is fully involved in all aspects of what is going on around them and ensure that they are facilitated to communicate and participate as much as possible.
- The Support Worker will encourage the client to act as independently as possible and will endeavour to alleviate any anxieties they may have through full explanation of what is going on at any time, explaining choices available and the possible outcomes.
- The Support Worker will liaise with the wider team, in developing ideas for appropriate social and home-based activities/pastimes, including developing the associated plans and risk assessments.
- The support worker with assist with general household chores and maintenance as detailed in the working file.
- The Support Worker may also be required to support the client on holidays both in the UK and abroad. This will be discussed on an individual basis and plans and agreements will be discussed well in advance.
