That's why we're looking for a Care Co-ordinator who wants more than just a job.

This isn't just about rosters
Of course, you'll be responsible for co-ordinating care and ensuring services run smoothly.
But the role goes far beyond scheduling.
You'll be the link between our clients, families, care team and healthcare professionals. You'll help us maintain high standards, solve problems, build relationships and continually improve what we do.
And because we're a relatively small service, you'll never be hidden away behind a desk.
When the situation demands it, you'll be prepared to roll up your sleeves and support care delivery directly. Whether that's covering a visit, helping during periods of increased demand or supporting the wider team, you'll understand that outstanding care is a shared responsibility.
What you'll be doing
- Creating and managing care staff schedules and rosters
- Completing new client assessments
- Conducting regular care reviews and quality assurance visits
- Carrying out audits and supporting service improvement initiatives
- Building positive relationships with clients and families
- Supporting and communicating with care staff
- Liaising with GPs, pharmacies, district nursing teams, therapists and other professionals
- Monitoring service quality and identifying opportunities for improvement
- Supporting recruitment, induction and staff development activities where required
- Providing hands-on care support when operationally necessary
- Helping ensure the service remains safe, effective, caring, responsive and well-led
- Supporting the manager with the day-to-day.
Who we're looking for
You'll be organised, proactive and genuinely passionate about delivering great care.
You won't be afraid to make decisions, solve problems or take ownership.
You'll enjoy building relationships and will understand that great care is about people, not processes.
Most importantly, you'll be somebody who sees opportunity where others see obstacles.
We'd particularly like to hear from people who:
- Have experience in domiciliary care, care co-ordination, senior care work or a similar health and social care role
- Understand person-centred care and regulatory requirements
- Communicate confidently with staff, families and healthcare professionals
- Can manage competing priorities in a fast-moving environment
- Lead by example and aren't afraid to get involved when needed
- Want to grow professionally as the organisation grows.

Pay: £26,000.00-£30,000.00 per year
