We provide person-centred, outcome-focused support for adults with complex neurological conditions, acquired brain injuries, stroke, neurodegenerative conditions, autism, learning disabilities, and complex mental health needs.

We are currently seeking an experienced and compassionate Team Leader to join our growing team and help deliver exceptional care and rehabilitation support to the people we serve.
The Role
As a Team Leader, you will lead by example, providing high-quality care and support whilst overseeing the day-to-day delivery of services within the unit. You will support and mentor care staff, promote best practice, and work collaboratively with healthcare professionals, therapists, residents, and families to ensure outstanding outcomes.
You will play a key role in creating a positive, therapeutic environment that promotes independence, dignity, choice, and recovery.
Key Responsibilities
- Lead and supervise care and support staff during shifts.
- Deliver person-centred care and rehabilitation support in line with individual care plans.
- Promote independence, recovery, and meaningful engagement in daily activities.
- Support individuals with neurological conditions, mental health needs, and complex behaviours.
- Ensure safe administration and management of medication where appropriate.
- Monitor and report changes in residents’ physical, emotional, and mental wellbeing.
- Contribute to risk assessments, care planning, and rehabilitation goal setting.
- Provide mentorship, guidance, and support to less experienced team members.
- Ensure compliance with CQC standards, safeguarding requirements, and organisational policies.
- Work collaboratively with nurses, therapists, social workers, and external professionals.
- Maintain accurate documentation and electronic care records.

About You
We are looking for a caring, motivated, and professional individual who shares our commitment to delivering outstanding care and rehabilitation outcomes.
Essential Requirements
- Minimum 2 years' experience in a senior care, support worker, rehabilitation support worker, or team leader role.
- NVQ/QCF Level 3 in Health & Social Care (or equivalent) or working towards.
- Experience supporting individuals with complex care needs.
- Excellent communication and leadership skills.
- Ability to work effectively as part of a multidisciplinary team.
- Strong understanding of person-centred care and positive risk management.
- Passion for promoting independence, dignity, choice, and recovery.
- Right to work in the UK.
Desirable
- Experience working within neurological rehabilitation, brain injury, stroke services, mental health services, supported living, or complex care settings.
- Knowledge of autism, acquired brain injury, challenging behaviour, or mental health conditions.
- Medication administration training.
- Experience of supervising and developing staff teams.
