Mental Health and Wellbeing Coach
Gwent | £30,018.46 per annum | Full-time, 37.5 hours

Join our client’s dynamic team as a Mental Health and Wellbeing Coach, where you'll have the opportunity to support individuals transitioning from secure or residential settings into independent community living. This role is vital in providing personalised, compassionate care and fostering positive change.
Key Responsibilities:
- Deliver practical support in areas such as support plan reviews, medical appointments, community activities, emotional regulation, and life skills development, including money management and tenancy support.
- Advocate for service users, ensuring they have choice and control over their support, promoting their independence and well-being.
- Develop tailored support plans rooted in Structured Clinical Management (SCM) principles, working collaboratively with the team and relevant health services.
- Maintain accurate records, incident reports, and escalate safeguarding concerns promptly.
- Engage in reflective practice, team meetings, and continuous improvement initiatives.
- Gather feedback through measurements and resident insights to review and enhance the quality of support provided.
Essential Requirements:
- Proven experience supporting individuals with mental health needs or challenging behaviour, ideally across a broad geographical area.
- A full driving licence and the ability to travel across Gwent and to inpatient settings further afield.
- Flexibility to work on a six-week rolling rota, including days, weekends, evenings, and nights.
- Ability to build trusting, therapeutic relationships, setting appropriate boundaries with service users.
- Strong emotional intelligence — non-judgmental, resilient, and empathetic communication skills.
- Capability to work autonomously, respond swiftly to individual needs, and adapt to changing priorities.
Desirable Skills & Qualifications:
- Knowledge of PIE, ACEs, and restorative approaches.
- Experience advocating for service user needs and working within community and third-sector organisations.
- Welsh language skills.
