The Role
As Care Coordinator, you will play a key role in the day-to-day running of the branch, ensuring care is delivered safely, efficiently and in accordance with individual care plans.
Working closely with the Registered Manager, Deputy Manager and Field Supervisors, you will coordinate care packages, support care staff, liaise with healthcare professionals and families, and help ensure the service remains fully compliant with CQC standards.
Main Responsibilities - Care Coordination
- Produce and manage efficient staff rotas using the electronic scheduling system.
- Ensure all care visits are allocated appropriately and delivered safely.
- Respond quickly to staff absences, emergency changes and new care packages.
- Maintain continuity of care wherever possible.
- Monitor Electronic Call Monitoring (ECM) and investigate late, missed or incomplete visits.
- Ensure all scheduled visits are accurately recorded.
Service User Management
- Undertake initial care assessments.
- Develop person-centred care plans.
- Complete and review risk assessments.
- Carry out regular care reviews.
- Update care documentation following changes in needs.
- Ensure all care records remain accurate and compliant.
Staff Support
- Act as a first point of contact for care staff during office hours.
- Provide day-to-day support and guidance to care staff.
- Liaise with Field Supervisors regarding spot checks, competencies and service user concerns.
- Assist with the allocation of emergency cover.
- Promote good communication and teamwork across the branch.
Quality & Compliance
- Support the Registered Manager in maintaining compliance with CQC Fundamental Standards.
- Ensure care documentation is accurate and up to date.
- Report and escalate safeguarding concerns in line with company procedures.
- Record and report incidents, accidents and complaints appropriately.
- Support branch audits and quality assurance activities.
- Promote safe medication practices and escalate medication concerns where required.
Communication
- Build positive relationships with service users and their families.
- Liaise professionally with Social Workers, Local Authorities, District Nurses, GPs, hospitals and other healthcare professionals.
- Respond promptly and professionally to telephone and email enquiries.
- Work collaboratively with the wider office team.

- Experience producing and managing staff rotas.
- Experience using Electronic Call Monitoring and scheduling systems.
- Experience completing care plans and risk assessments.
- Good understanding of CQC Fundamental Standards.
- Good understanding of safeguarding procedures.
- Excellent organisational and time management skills.
- Ability to prioritise and manage multiple tasks in a fast-paced environment.
- Excellent communication and interpersonal skills.
- Strong IT skills.
- High attention to detail.
- Professional and compassionate approach.
- NVQ Level 3 Diploma in Health and Social Care (or above).
- Full UK driving licence and access to your own vehicle.
Desirable
- Experience using electronic care planning and scheduling systems.
- Experience liaising with Local Authorities and NHS professionals.
- Experience supporting quality assurance and compliance activities.
- Knowledge of domiciliary care legislation and best practice.
