About the Role
We are looking for an organised and compassionate Area Care Co-ordinator to join our Market Weighton team. This is a vital role within our service, ensuring the smooth coordination of care delivery and supporting high-quality, person-centred care in line with CQC standards and company values of Trust, Respect, Excellence and Kindness.
You will play a key role in scheduling care, supporting staff, responding to day-to-day operational challenges and ensuring service users receive safe, effective, and responsive support.
Responsibilities
- Coordinating and allocating care calls using digital systems to ensure efficient service delivery
- Ensuring service user records and notes are accurate and up to date
- Monitoring and responding to alerts, incidents, and staffing needs
- Communicating effectively with staff, service users, families, and external professionals
- Supporting referrals with our assessment team and other healthcare professionals such as GPs and District Nurses
- Participating in the on-call rota, including evenings and weekends
- Providing emergency cover and responding to urgent care needs when required
- Managing incoming referrals and telephone enquiries
- Supporting the wider team to maintain a safe, responsive, and well-led service
Skills
- Excellent communication and organisational skills
- Experience in care coordination, scheduling or health and social care
- Ability to work in a fast-paced environment and manage competing priorities
- Understanding of person-centred care and CQC standards
- Strong problem-solving skills and ability to respond to challenges effectively
- A flexible and proactive approach to work
- A valid UK driving licence and access to a vehicle essential
