Moordown Medical Centre is a busy, forward-thinking GP practice and an active member of Central Bournemouth Primary Care Network (PCN). We are looking for an enthusiastic, organised and patient-focused Care Co-ordinator to support the delivery of high-quality healthcare services across our practice and PCN.
This is an exciting opportunity for someone who enjoys a varied role, working with patients, clinicians and partner organisations to help ensure patients receive the right care, at the right time, from the right service.
About the Role
As a Care Co-ordinator, you will play a key role in supporting patients and the clinical team by coordinating appointments, referrals and healthcare initiatives across the practice and Primary Care Network.
You will work closely with GPs, nurses, allied health professionals and management teams to support a range of services, helping improve patient access and ensuring efficient delivery of care.
Key Responsibilities
· Booking appointments for a range of PCN services including First Contact Physiotherapists, Mental Health Services, Pharmacy Teams and Extended Access clinics.
· Managing blood test requests and supporting GP’s with medication review monitoring.
· Supporting online consultation workflows and patient navigation including completion of BaRs referrals to community pharmacies.
· Assisting with Quality and Outcomes Framework (QOF) work and patient recall programmes.
· Coordinating vaccine ordering and delivery arrangements for vaccination clinics.
· Processing Smartcard access requests and supporting PCN administrative functions.
· Processing PCN invoices and supporting financial administration activities.
· Working with patients to encourage engagement with healthcare programmes and preventative care initiatives.
· Support with PCN BP Lucii monitoring in line with the PCN BP protocol
· To undertake any other duties or projects commensurate with the nature and grade of this post required.
· Responsible for promoting and safeguarding with welfare of children and vulnerable patients that you come in contact in line with practice procedures and policies.
About You
We are looking for someone who:
Essential
Has previous experience in a GP Practice, Primary Care, NHS or healthcare administration environment.
Has excellent organisational and administrative skills.
Can manage competing priorities and work independently.
Has strong IT skills and is comfortable learning new systems.
Can confidently multi-task
Possesses excellent communication and interpersonal skills.
Works effectively as part of a team.
Understands the importance of confidentiality and data protection.
Desirable
Experience using SystmOne.
Knowledge of QOF, patient recalls or referral pathways.
Experience supporting healthcare projects or vaccination programmes.
Understanding of Primary Care Networks and NHS services.

Pay: £25,350.00-£25,900.00 per year
