We are seeking an experienced Practice Nurse to join a friendly GP practice team on a locum maternity cover contract from June 2026 for 6 – 12 months.
The role will primarily focus on Long-Term Condition (LTC) reviews, delivering high-quality, patient-centred care within a structured primary care setting.
This is an excellent opportunity for a confident and autonomous Practice Nurse looking for flexible working and a stable long-term placement.

Key Responsibilities
You will be responsible for conducting combined LTC clinics, including:
- Asthma reviews
- COPD management reviews
- Diabetes annual reviews
- Hypertension monitoring and management
Requirements
- Registered Nurse with valid NMC pin
- Experience working as a Practice Nurse in primary care
- Strong experience in LTC reviews (Asthma, COPD, Diabetes, Hypertension)
- Ability to work independently in a GP setting
- Excellent communication and patient care skills
- Confident managing structured chronic disease clinics
