Rainbow Care Solutions is a CQC-registered provider delivering person-centred, outcomes-focused care. We are recruiting an Occupational Therapist to support our Discharge to Assess (D2A) / Reablement service, helping people regain independence at home following hospital discharge. You’ll work as part of a multi-disciplinary team, completing timely assessments, setting practical goals, and supporting a “doing with, not doing for” approach that promotes confidence, safety, and measurable progress.
Key responsibilities
- Complete OT assessments in the community and contribute to discharge planning
- Develop and review reablement plans with clear, measurable goals
- Provide guidance to reablement/support staff on safe techniques and enablement approaches
- Recommend equipment and minor adaptations, and support onward referrals where needed
- Participate in MDT meetings, case reviews, and service improvement activity
- Maintain accurate, timely documentation in line with governance and GDPR requirements
- Support risk management, safeguarding, and quality assurance processes
What we’re looking for
- HCPC-registered Occupational Therapist
- Experience in reablement, discharge, intermediate care, community OT or similar
- Strong understanding of enablement and strengths-based practice
- Confident working autonomously across a rural/countywide patch
- Excellent communication skills and a collaborative MDT approach
- Full UK driving licence and access to a vehicle (essential)
