An exciting opportunity has become available for an Advanced Practitioner in Social Work to work within the Prevent, Reduce and Enable Service, based within the East Adult Contact Team at Macclesfield Town Hall. The position is full time 37 hours a week, from Monday to Friday.
About the role
This position is to support the Adult Social Care Teams in the East on the Prevent, Reduce and Enable Team, to provide a quick and urgent response to those individuals who have been referred with a presenting need. this is a fast-paced role working alongside a small team of Social Care staff and Occupational Therapy and the Reablement Service. The aim of this service is to prevent further crisis or deterioration in needs and preventing a hospital admission.
The focus for the Advanced Practitioner is on ensuring that practice standards are maintained and that assessments are strengths-based and person-centred, whilst ensuring that interventions represent a proportionate response to presenting needs. The Advanced Practitioner will work closely with the Practice Manager and focus on quality assurance for practice related issues within the teams through supervision and informal support for staff. The Advanced Practitioner will give guidance to staff to support their decision-making and to help them tackle challenging conversations and conflict. The Advanced Practitioner would not be expected to deal with budgetary, staffing or team performance issues but is likely to hold a small caseload and deal with any related safeguarding concerns.

You will have:
Extensive knowledge of the Care Act 2014 and understand Cheshire East Polices & Processes
A high degree of interpersonal skills
To oversee effective teamwork and have excellent communication skills
Have the ability to converse at ease with both internal & external partners and customers
Have the ability to work flexibly
Be able to provide formal supervision to an identified group of staff
Be able to oversee and manage safeguarding concerns and high-risk situations.
