Job Overview
We are seeking a dedicated and experienced Lead Practitioner to oversee and coordinate care services at CM Bespoke Care Ltd. The ideal candidate will possess strong leadership qualities, excellent communication skills, and a thorough understanding of home care practices. This role involves guiding care staff, ensuring the delivery of high-quality support, and maintaining compliance with care plans and regulatory standards. The Lead Practitioner will play a vital role in fostering a compassionate environment where clients’ needs are prioritised and met with professionalism.
Duties
- Lead and supervise care staff to ensure the delivery of personalised care in accordance with individual care plans.
- Ensure smooth operations and high standards of service.
- Assist clients with daily living activities, promoting independence whilst providing necessary support.
- Develop, review, and update care plans in collaboration with clients, families, and healthcare professionals.
- Ensure compliance with health and safety regulations, maintaining a safe environment for clients and staff.
- Provide guidance and mentorship to junior staff members, fostering professional development.
- Maintain accurate documentation related to domiciliary care and operational procedures.
- Communicate effectively with clients, families, external healthcare providers, and team members to ensure cohesive service delivery.
- Support the recruitment process by participating in interviews and training new team members as required.
- Utilise IT systems to record data accurately and efficiently manage client information.
- Be a part of an on call rota to deal with emergencies outside of office hours.
Requirements
- Proven experience working within a home care environment, at least a Diploma Level 2 in Health and Social Care, preferably Level 3.
- Strong communication skills in English, both verbal and written.
- Ability to develop and implement effective care plans tailored to individual needs.
- Driving licence is desirable for facilitating transport or home visits.
- Good IT literacy skills to manage electronic records and documentation efficiently.
- Excellent organisational skills with the ability to lead a team effectively under pressure.
- Compassionate approach with a genuine commitment to providing high-quality resident-centred care.
