City South PCN 7 are looking for a social prescribing link worker , this is an exciting opportunity to join our team on a fixed term basis.
Social prescribing link workers will work as a key part of the primary care network ( PCN ) multidisciplinary team promoting independence , wellbeing and knowledge . Social prescribing can help pcn's strengthen community and personal resistance , reduce health inequalities and wellbeing inequalities .
Signposting to relevant services to support , poor housing , debt and physical inactivity .and be the community link.
The social prescribing role provides GP with a non medical referral option that can align to existing treatment's to improve Health and wellbeing .
The role will provide information and support to patients in addition to becoming the link between the patient, GP and other service providers. The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals.
Responsibilities:
Is an effective and trusted member of the Multi-Disciplinary Team (MDT)
Maintains compliance with mandatory and statutory training
Understands obligation to safeguard individuals from harm
Understands and maintains compliance with information governance principles and protocols
Understands obligations to keep and maintain accurate records
Utilises available evidence base for social prescribing interventions and activities
Understands the social/wider determinants of health, health inequalities and population health
Records and demonstrates impact through multiple routes
