Care Coordinator - Peterlee
Location: Traynor Business Park, Peterlee, SR8 2RU
Salary: £30,000 + bonuses
Additional: Out-of-hours on-call (paid)

About the Role
We are looking for a compassionate, organised, and experienced Care Coordinator to join our dedicated team. As a key member of our service, you will ensure that clients receive seamless, high-quality care that meets their individual needs. This is a rewarding role where you can truly make a difference in people’s lives.
Key Responsibilities
- Coordinate personalised care plans by liaising with healthcare professionals, clients, and families
- Schedule appointments and maintain strong communication with key stakeholders
- Keep accurate, up-to-date records of client information and care plans
- Monitor and review the effectiveness of care delivery
- Provide emotional support and guidance to clients and their families
- Work collaboratively with multidisciplinary teams to ensure holistic care
- Line-manage a small team of care workers across Peterlee, Murton, Seaham, Blackhall and surrounding areas
- Create and manage staff rotas for community-based care
- Participate in an out-of-hours on-call rota (additional pay)
About You
- Previous experience in care coordination or a similar role
- Excellent communication and interpersonal skills
- Strong organisational abilities with the capacity to prioritise and multitask
- Competent in Microsoft Office and general administrative software
- Compassionate, empathetic, and committed to providing exceptional care
- Full UK driving licence and access to a car for work-related travel
- Willingness to visit clients in the community as required
