Role Purpose: The vision of this service is to achieve a holistic, innovative and flexible service to support carers across Carmarthenshire to improve and maintain their health, well-being and independence through social integration. The role offers an exciting opportunity to be involved in the development and delivery of Adferiad’s Carers Services across Carmarthenshire, through co-production and partnership working. The post-holder will work flexibly to engage with carers to ensure their voice is at the heart of the development. The post-holder will work in a holistic, strength based, recovery focused way, fully engaging with the carer to identify what their needs are and what matters to them. Support provided will focus on promoting independence, building resilience and promoting community engagement. The post-holder will help individuals recognise their achievements, skills and strengths and use these to help them identify and work towards future goals.
