An exciting opportunity has arisen for an experienced End-Stage Chronic Liver Disease Clinical Nurse Specialist to join our Gastroenterology Directorate.
As an IQILS-accredited tertiary hepatology centre, you will work as a lone CNS within a supportive multidisciplinary hepatology team, providing expert care for patients with end-stage chronic liver disease. You will lead on specialist assessment, care coordination, advanced care planning, patient and family support, and service development to ensure high-quality, evidence-based care.
Applicants must have substantial hepatology experience and previous Clinical Nurse Specialist experience.
This is a 2-year fixed-term, charity-funded post.

As the End-Stage Chronic Liver Disease Clinical Nurse Specialist, you will provide expert clinical leadership and specialist care for patients with advanced liver disease. You will undertake comprehensive assessments, coordinate complex care, lead on advance care planning, and provide specialist advice, education and support to patients. Working closely with hepatology, ward teams, the wider multidisciplinary team and community palliative care services, you will ensure patients receive seamless, high-quality, person-centred care throughout their pathway.
You will also play a key role in developing and improving the service through clinical audit, research, education and the implementation of evidence-based practice. The post holder will contribute to the development of policies, protocols and clinical guidelines, supporting continuous service improvement while promoting patient safety, dignity and the highest standards of care.

Within this directorate, the role of the End-Stage Chronic Liver Disease Clinical Nurse Specialist is central to the coordination and delivery of high-quality patient care. The CNS provides expert clinical knowledge, leadership and support across the patient pathway for patients and their families.
1. Clinical Care
Assess, plan, implement, and evaluate care for patients with End-stage liver disease
Facilitate advance care planning
Support symptom control and palliative care needs
Monitor patients awaiting or unsuitable for liver transplant
2. Care Coordination
Act as a key worker for ESLD patients across inpatient and outpatient settings
Liaise with multidisciplinary teams (hepatology, dietitians, palliative care, social services)
Facilitate discharge planning and community follow-up
3. Patient & Family Support
Provide education on disease progression, medications, and lifestyle changes
Support patients and families with complex decision-making and advance care planning
Offer psychological and emotional support
4. Education & Training
Educate nursing staff and junior clinicians on liver disease management
Develop and deliver teaching programs
Promote best practices in hepatology nursing
5. Leadership & Service Development
Lead initiatives to improve ESLD care pathways
Participate in audits, research, and quality improvement projects
Contribute to policy and guideline development
6. Research & Evidence-Based Practice
Apply current research to clinical practice
Participate in clinical trials or liver disease research where applicable

8. Qualifications & Experience
Registered Nurse (RN) with valid license
Master’s degree or working toward advanced practice qualification
Significant experience in hepatology, gastroenterology, or chronic disease management
Experience in palliative or end-of-life care preferred
