Key Responsibilities:
- Undertake assessments, care planning, and interventions for children with SEND
- Lead direct work with families, creating tailored support plans
- Deliver early help and preventative interventions to reduce escalation
- Coordinate multi-agency working to ensure holistic, child-centred outcomes
- Apply knowledge of safeguarding, legislation, and child protection frameworks
- Implement the graduated approach to SEND support and intervention
- Support young people with preparing for adulthood, independence, and life skills
- Participate in reflective supervision and contribute to service development
Requirements:
- Qualified Social Worker with Social Work England registration
- Experience working with children with disabilities / SEND
- Strong understanding of safeguarding and UK social work legislation
- Ability to manage complex caseloads and multi-agency coordination
- Excellent communication and relationship-building skills
