The post holder will be part of the 0–19 Health and Wellbeing Service, delivering public health advice, information and guidance, with a strong focus on early intervention and prevention. They will contribute to the assessment, planning, delivery and evaluation of health programmes, working within their scope of competence and professional knowledge.
Working in line with professional codes, standards and evidence-based practice, the post holder will promote the health and wellbeing of children and young people aged 0–19 across Stoke-on-Trent, supporting them to achieve their developmental and educational potential.
The 0–19 Children, Young People and Families Practitioner will work collaboratively within universal- and multi-disciplinary teams to address a wide range of health, care and support needs. They will contribute to the delivery of the Healthy Child Programme (0–19) and support improved population health outcomes, in line with local service specifications.
The role involves working across a variety of settings, including Family hubs, providing advice, guidance, signposting and referrals to appropriate services.
The successful applicant will across the city in various locations supporting children, young people and their families.
Develop and maintain effective partnerships across the Trust and with external agencies, promoting equality and inclusion
Deliver services as part of the 0–19 Health and Wellbeing Service to meet the needs of local children and families
Identify public health needs and support delivery against national and local priorities
Plan and deliver health promotion activities in communities, schools and partner settings
Follow safeguarding procedures and report concerns in line with local policy
Support children within Child Protection and Looked After systems, contributing to required reports
Promote key health priorities including immunisations, oral health, emotional wellbeing, healthy lifestyles and school readiness
Engage and involve children, young people and families in care planning and interventions
Implement care plans and support children with additional health needs, contributing to EHC plans where appropriate
Maintain accurate records and apply sound professional judgement, escalating when required
Apply frameworks such as Fraser Guidelines within scope of practice
Manage and prioritise workload under the guidance of a registered practitioner
Support care plans that promote independence, wellbeing and reduce health inequalities

The post holder will work as part of the 0–19 Children and Young People’s Health and Wellbeing Service within Midlands Partnership NHS Foundation Trust, delivering high-quality public health interventions to children, young people and families across Stoke-on-Trent. The role focuses on early intervention, prevention and health promotion, supporting improved outcomes in line with local and national priorities, including the Healthy Child Programme (0–19).
Working collaboratively within universal- and multi-disciplinary teams, the post holder will contribute to the assessment, planning, implementation and evaluation of care and support programmes. They will engage children, young people and families in this process, promoting empowerment, independence and self-management. The role involves working across a variety of settings, including homes, schools, community hubs and clinics, providing advice, guidance, signposting and referrals to appropriate services.
The post holder will support delivery of key public health priorities such as immunisations, oral health, emotional wellbeing, school readiness, healthy weight and nutrition, smoking cessation, sexual health and supporting vulnerable groups. They will identify health needs within individuals and communities and contribute to achieving agreed targets and outcomes.
Safeguarding is central to the role, requiring adherence to local and national procedures, including identifying and reporting concerns and supporting children within Child Protection and Looked After systems. The post holder will also contribute to reports and care planning processes, including Education, Health and Care Plans where appropriate.
Operating within professional codes, standards and evidence-based practice, the post holder will maintain accurate records, use sound clinical judgement and seek support from registered practitioners when needed. They will prioritise workload effectively and build strong working relationships with colleagues and partner organisations, ensuring services are inclusive and responsive to the diverse needs of the population served.
