The post holder will provide high-quality occupational therapy assessment, intervention and rehabilitation to patients across a range of community settings, including care homes, residential units and individuals’ own homes.
You will work closely within a multidisciplinary team to support admission avoidance, facilitate early hospital discharge and promote independence and recovery.
Key responsibilities include:
Undertaking holistic assessments and delivering evidence-based interventions tailored to individual needs.
Prescribing, ordering and providing education on the safe use of equipment and adaptations.
Contributing to effective care planning and risk management to support safe care at home.
Supervising, mentoring and supporting the development of junior staff and students.
Building strong, collaborative relationships with patients, families, carers, colleagues and partner agencies.
You will be expected to work flexibly and contribute to the delivery of a responsive, patient-centred service that meets the needs of the local population.
