Topioneer Care Ltd is a dedicated and person-centred domiciliary care provider committed to delivering high-quality care and support services to individuals within their own homes. We are seeking a highly motivated, experienced, and organised Care Coordinator to join our Goole Branch and play a key role in ensuring the efficient delivery of care services.
Key Responsibilities
- Coordinate and manage care staff rotas to ensure effective service delivery.
- Schedule and allocate care visits while maintaining continuity of care.
- Liaise with clients, families, healthcare professionals, and local authorities.
- Support the recruitment, induction, and supervision of care staff.
- Monitor care delivery and ensure compliance with CQC standards and company policies.
- Maintain accurate records, care plans, and electronic care management systems.
- Respond promptly to emergencies, service changes, and staffing issues.
- Assist with quality assurance activities, audits, and service improvements.
- Participate in the on-call rota as required.
Essential Requirements
- Minimum of 2 years' experience in a Care Coordinator, Senior Care Coordinator, Field Care Supervisor, or similar role within domiciliary care.
- Strong knowledge of CQC regulations and care sector compliance requirements.
- Excellent organisational, communication, and problem-solving skills.
- Experience using care scheduling and rostering software.
- Ability to work effectively under pressure and manage multiple priorities.
- Competent in Microsoft Office applications.
- Full UK Driving Licence and access to a vehicle (preferred).
Desirable Qualifications
- NVQ/QCF Level 3 in Health and Social Care (minimum).
- Level 5 Diploma in Leadership for Health and Social Care or willingness to work towards it.
- Experience working with local authority and NHS-funded care packages.
