We are looking for nurses with a background within invasive and non-invasive ventilation with experience with adult clients to be responsible for overseeing a clinical caseload of clients living within their own homes. Leading and training a team of support workers in the delivery of complex care packages within the community. Provide clinical expertise and professional leadership to enable management of a complex caseload of clients, requiring clinical support in the community and to oversee the planning and delivery of safe, high quality complex care to adults in their own homes
In this post you will provide highly skilled, quality assessments that are outcome focused, to support our clients and their families, enabling them to live, participate and achieve in all aspects of life, e.g. at home and within their community. The workload is varied and interesting with opportunity to develop further knowledge and skills.
Purpose of the role:
- Manage the clinical care requirements of existing and new clients
- Mentor and support a team of support workers
- Ensure new packages are implemented safely and in a timely manner
- Develop working relationships with clients, their families and health and social care professionals
General Duties
- Assess potential new clients within their current setting (Home, Hospital, Hospice, Care Home, Respite)
- Set up new complex care packages, including writing care plans and risk assessments, training staff on clients health needs, organising any specialist training, induct new Support Workers, mentor and teach new and existing staff
- Support in the mobilisation and implementation of complex care clients to support with business growth
- Provide evidence based clinical expertise and advice relating to care-planning and delivery across the pathway and evaluate outcomes
- Attend meetings and/or liaise with MDT, commissioners, case managers and clients etc to build and manage clinical caseload
- To facilitate and deliver an ongoing programme of clinical knowledge/skills-training and competency assessments to non-registered members to enable them to competently deliver care with minimal supervision
- To be responsible and accountable for the delegation of clinical healthcare tasks required for clients to maintain a quality of life within the community
- Maintain high standards and proactively manage quality improvement, clinical risks, safety concerns and complaints relating to client case load
- Promote compliance and adherence to internal policies and standards
- Weekly visits to clients to ensure package of care is running smoothly as per care plan
- Review and update care plans and risk assessments
- Assess Support Workers practical Skills to ensure competence and safe working practices
- Complete regular medication and care plan audits
- Respond to complaints, Safeguardings and concerns
- Complete weekly complex care reports
- Collaborate with healthcare professionals for comprehensive client care
- Maintain precise client records including medical histories and treatment plans
- Exceptional leadership skills with the ability to effectively groups of small teams
About you
- A minimum of 3 years post registration (RGN and valid NMC pin)
- ICU, HDU or A&E experience essential
- Experience within Complex Care – on intensive care (or similar), or within the community
- Demonstrable management of clinical risk management and individualised, person-centred care planning
- Experience of safeguarding adults
- Develop own knowledge and skills and that of others
- Promote best practices as an ITU Nurse – RGN in health and safety and security.
- Work autonomously with minimum supervision
- Excellent IT and presentation skills
- Driver with own vehicle
- Excellent communication skills and ability to be able to train clinical and non-clinical staff
