To provide consistent, high-quality care and support for two female, adult clients, M and N, who both have cerebral palsy and differing care and support needs. Each requires 24-hour assistance to remain safe, comfortable, and well cared for within their home environment.
Support Workers will deliver person-centred care that recognises and respects each individual’s unique needs, preferences, and routines. The role involves supporting all aspects of daily living, including personal care, mobility, medication administration, communication, and engagement in meaningful activities.
You will work closely with other team members, therapists, and family, under the direction of the Case Manager / Care Co-ordinator, to ensure continuity of care, promote wellbeing, and encourage independence wherever possible.
We are seeking motivated, reliable, and compassionate Support Workers to join an established team providing 24/7 care for M and N within their home.
Both clients have cerebral palsy but require differing approaches to meet their individual needs. Support will include:
· Personal care and hygiene support, respecting privacy and dignity at all times.
· Assistance with mobility, transfers, and the safe use of hoisting and other moving and handling equipment.
· Supporting communication through the use of appropriate aids, patience, and understanding.
· Medication administration in accordance with prescribed protocols and EMAR systems.
· Supporting nutrition and hydration in line with individual care plans.
· Promoting engagement in daily routines, leisure interests, and community participation.
· Encouraging independence, self-expression, and personal choice.
· Maintaining a clean, safe, and positive home environment.
You will work as part of a small, skilled team providing round-the-clock support, including day shifts, waking nights, and sleep-in duties. Flexibility is essential to maintain consistency of care.
