We are seeking a skilled and motivated Occupational Therapist to join our Locality Therapy Team. This post will be based with our Community Therapy team , now working out of Ludlow Hospital.
The successful individual will join an enthusiastic and supportive multi-disciplinary team, including Occupational Therapists, Physiotherapists, Assistant Practitioners and Community Nurses in providing high quality care and rehabilitation to patients in their own homes.
The post holders’ main responsibilities will be providing Occupational Therapy assessment and treatments to patients who have a wide range of conditions and complex needs, on multiple pathways into the service. The successful individual must have an interest and experience within the Community setting and be able to work as an autonomous practitioner, managing their own caseload.
There is an opportunity to work into our two Community Hospitals in the South West locality, to develop or maintain clinical skills and support patients on their journey through our services.
We have a strong commitment to clinical supervision and Continuing Professional Development (CPD), offering a supportive and proactive environment with opportunities for supervision, training, and peer support, both locally and County wide. This post will be involved in supervision of junior staff, assistants, and students.

To provide a high quality seamless Occupational Therapy service from Acute or Community Hospital to Community, responsive to the needs of the local population as part of a cohesive multidisciplinary team.
To work as a key member of the multidisciplinary team (MDT) that is responsive to the needs of the local population, working flexibly to respond to the changing needs of the patient.
To provide a key role in facilitating the seamless transition from community hospital to patient’s own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life.
To provide supervision, facilitate the personal and professional development of junior members of staff, enhancing their skills and knowledge and where appropriate line management.
