This is an exciting opportunity to join us at the very start of the Early Supported Stroke Discharge (ESSD) service for High Peak and North Dales .
About the role
We are seeking a skilled, motivated and forward ‑ thinking Band 7 Occupational Therapist to play a key clinical and leadership role within our newly established ESSD team.
Alongside delivering specialist, evidence ‑ based stroke rehabilitation in the community, you will provide clinical leadership , supporting and deputising for the ESSD Team Lead where required . You will help shape service delivery, support quality improvement, and contribute to the development of staff within the team, ensuring excellent outcomes for patients and carers.
This post aligns with the established ESSD model across Derbyshire and offers the opportunity to influence how the service develops from day one.
To lead and support delivery of the Occupational Therapy Service provided to Stroke patients in the ESSD Service, and evaluate this, using this working knowledge to help influence developments within the Community Stroke pathway
To be responsible for the specialist education, training and supervision of staff treating patients with stroke in the ESSD Service, as well as the wider Stroke and Neuro Service where required.
To work collaboratively with the other Clinical Specialists within the ESSD and wider Stroke and Neuro Service to help drive forward developments and evidence based practice.
To hold a clinical caseload within the ESSD Service
To act as a highly specialist, autonomous clinician with advanced theoretical and practical knowledge and skills; providing a very high standard of effective, safe, timely evidence-based education and care for stroke patients.
To work in partnership with primary, secondary care and other agencies to facilitate early discharge of patients and prevent hospital re-admission.
To lead and participate in audit and research within the service (including SSNAP)
To support and deputize for the ESSD Teams Lead where required.
