Role will be a hybrid model based partly working from the office at NW6 6BS with the requirement to visit approximately six properties under IHL management on a regular basis. Ideal candidate will live in North London. Mileage paid for travel expenses.

Key Responsibilities:
To ensure that the service provides a safe, client focused environment that utilises a holistic approach to support.
- Provide practical and emotional support to clients; giving guidance on issues as they arise.
- Provide a holistic approach in supporting clients to achieve the goals outlined in their support plan.
- Effectively engage and empower clients to review choices and behaviours and make positive changes to their lifestyle; working in partnership with other services as appropriate.
- To support the client to maintain their tenancy and to develop the skills required to live independently in the future.
- Work with The Local Authority and certain charities to assist the client with move on at the point that they are ready for service exit. To work collaboratively with Housing Officer and Project Co-Ordinator
- To assess needs and risk in partnership with the client; ensuring that support and risk management plans are robust.
- To regularly review support management plans ensuring that the support aims are in line with the clients needs and wishes, and that the level of support provided is suitable.
- To complete Housing Outcome stars with all tenants, to update as a live document and complete quarterly updates with the tenants.
- Work in partnership with the housing management team to ensure best possible outcomes for customers.
- Ensure that all required documentation is accurately maintained and up to date.
- Ensure that safeguarding incidents, accidents and near misses are reported and responded to appropriately.
- Support the Project Co-Ordinator in ensuring that the service meets its key performance indicators, addressing any barriers appropriately.
- Provide data as required by the Project Co-Ordinator.
- Report all maintenance and health and safety concerns to the Maintenance team and support clients to do so. Using IHL's maintenance reporting system.
- Support and encourage clients to attend house meetings.
- Assist Housing officer and /or Project Co-Ordinator in responding to internal / external audit ensuring that any actions are completed in a timely manner.
- To participate in multi agency meetings providing feedback to teams and other managers as appropriate.
General

How will the post holder know if they are performing?
· A number of performance targets will be planned and agreed regularly at supervision meetings with your Area Manager, and these will be monitored at regular 121s. Regular feedback will be provided, and there will be an opportunity to discuss any problems, personal and team matters and development issues.

· The post holder will be expected to comply with IHLUK policies and procedures at all times. These include, but are not limited to equality and diversity, confidentiality, data protection and health and safety.
· In order to do this job, the post holder will be trained and coached in IHLUK procedures and policies. They will be expected to familiarise themselves with the procedures and policies and to seek advice and guidance from the housing manager if required.

Note
· Basic DBS checks are required for roles where you will be working with or have lone contact with Vulnerable Adults
· CIH Qualification or minimum of 2 years property or person-centred support related experience preferable, candidates would also benefit from experience working with a trauma informed practice approach.
