About the role:
Ashley Care are looking for an experienced, passionate and knowledgeable Trusted Assessor to carry out new client assessments, help to create person centred care plans, and carry out subsequent care plan reviews for our new and existing service users.
As a trusted assessor, you will be responsible for assessing our service users' health and social care needs, including risk assessments, and agreeing outcome focused care plans alongside them and their family members. You will need to be a great communicator, and able to respond to any changes of care, informing our team members and service users to ensure a smooth delivery of service.
Level 3 NVQ in Health and Social Care is desired, as well as up to date knowledge of care plan delivery, risk assessments, and safeguarding policies. You will work closely with our coordinators, and will develop relationships with other external community parties including occupation therapists and district nurses.
Key Skills:
- Hold or be working towards QCF level 3
- Substantial experience of working within a care environment
- Computer literate and a good eye for detail to ensure customer records are accurate and up to date
- Proactive – be able to forge relationships with health and community practitioners as well as finding referrals
- Networking and forging relationships to gain a full understanding of the Community and what offerings are available
- Experience with dealing with sensitive data and understanding of Information Governance
- Ability to interpret community care assessments into Service User plans
- Demonstrable knowledge of the CQC and legal requirements for community-based services, specifically for Domiciliary services
- Strong ICT skills with demonstrable experiencing of working with care planning systems and care assessment tools
- Ability to undertake role-based risk assessments and all associated paperwork which reduces/mitigates risk

What we're looking for:
- Level 3 NVQ in Health and Social Care desired but not essential
- Driving license: You must have a UK driving license and your own car with business insurance.
- Flexibility: You should be comfortable to work a mixture of daytimes, evenings, and alternate weekends.
