About the role
Are you passionate about patient safety, quality care, and making a real difference behind the scenes? Join our friendly and forward-thinking Quality Team at Ashgate Hospice as a Clinical Governance & Patient Safety Facilitator.
In this varied and rewarding role, you’ll work closely with the Head of Clinical Governance & Patient Safety to help coordinate key governance processes across the Hospice, including incident management, clinical risk, medicines safety, clinical audit, quality improvement and patient experience activities. You’ll play a key part in helping us maintain high standards, meet regulatory requirements, and continuously improve the care we provide.
No two days are the same. You might be coordinating a clinical audit, supporting an incident investigation, monitoring governance actions, preparing evidence for CQC, or working with teams to embed learning and improve patient care. You’ll work collaboratively with clinical teams, championing a culture of openness and learning.

What you’ll be doing:
You'll play a key role in supporting safe, effective and high-quality care across the Hospice by helping coordinate our clinical governance and patient safety activities.
- Supporting patient safety and governance – coordinating incident management processes, investigations, risk management, medicines governance, CAS alerts and coronial cases
- Driving clinical effectiveness and quality improvement – supporting clinical audits, benchmarking against national standards and helping ensure policies and documentation reflect best practice
- Monitoring improvement actions – tracking actions arising from incidents, audits, complaints and reviews to ensure improvements are implemented and sustained
- Supporting patient experience – working with colleagues to monitor feedback, complaints and improvement plans
- Promoting learning and development – sharing lessons learned, contributing to training and supporting a positive culture of openness, learning and continuous improvement
- Supporting governance and compliance – contributing to governance meetings, organisational reporting and CQC readiness activities
- Providing leadership support – deputising for the Head of Clinical Governance & Patient Safety when required

What you’ll bring:
You'll be a registered healthcare professional with experience of governance, patient safety, quality improvement or clinical audit activities, together with a passion for improving services and patient outcomes.
You’ll have:
- Professional registration with an appropriate healthcare regulatory body (NMC, HCPC, GMC or GPhC) and relevant post-registration experience
- Working knowledge of incident investigation processes, including PSIRF principles and a systems-based approach
- Understanding of human factors and patient safety, with the ability to apply this in practice
- Good knowledge of clinical governance, risk management, and regulatory standards (e.g. CQC)
- Experience supporting teams to improve patient safety and quality of care
- A commitment to just culture principles, encouraging openness, reflection, and learning
- Strong organisational, communication and analytical skills, with the ability to manage a varied workload and competing priorities
- Ability to work independently while contributing effectively within a multidisciplinary team
It would be great if you also have:
- Experience within hospice, community, NHS or voluntary healthcare services
- Experience of facilitating training, education or learning events
- Knowledge of quality improvement methodologies and clinical audit processes
- Experience of service improvement, change management or research activities
- Experience of supporting people with palliative, complex or life-limiting conditions
