Overview
We are seeking compassionate and dedicated Home Care Practitioners to provide high-quality support and assistance to clients in their own homes. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring assistance with daily activities, including elderly and disabled clients. The position involves working closely with clients to promote independence, safety, and well-being, ensuring they receive personalised care tailored to their needs. Prior experience in assisted living, nursing homes, or hospice care is highly valued. Duties
- Assist clients with personal care tasks such as bathing, dressing, grooming, and mobility support
- Provide medication administration as prescribed and monitor health conditions
- Support with meal preparation, feeding, and nutritional needs
- Engage clients in meaningful activities and behaviour management strategies
- Assist with light housekeeping duties to maintain a clean and safe environment
- Drive clients to appointments, errands, or social activities as required
- Monitor and report any changes in client health or behaviour to relevant healthcare professionals
- Support clients with dementia or Alzheimer’s care, ensuring their safety and comfort
- Offer emotional support and companionship to promote mental well-being
- Follow all health and safety protocols, including first aid procedures when necessary
Requirements
- Previous experience in assisted living, nursing home care, hospice care or home care is preferred
- Knowledge of dementia, Alzheimer’s care, and behaviour management techniques
- Ability to perform heavy lifting when necessary and assist with mobility support
- Valid driving licence for transportation duties
- First aid certification is desirable but not essential; training can be provided
- Strong communication skills with the ability to build trusting relationships with clients and their families
- Compassionate attitude with a genuine desire to improve clients’ quality of life
- Reliable, organised, and able to work independently or as part of a team
- Flexibility to work various shifts including mornings, afternoons, evenings, or weekends if required

Pay: £24,420.00-£52,343.16 per year
