We’re looking for a confident and experienced Transformation Manager to lead a focused 12‑month improvement programme across our Independent Lives homecare service.
This pivotal role will drive the development of a high‑quality, person‑centred home support offer, ensuring our customers receive safe, reliable and compassionate care that enables them to live independently at home.

The Role
As Transformation Manager, your focus will be on:

Leading a structured transformation programme to strengthen and modernise our Independent Lives, homecare operations

Embedding clear team structures, accountability and consistent ways of working across all Independent Lives, home support teams

Improving enquiry handling, assessment pathways and conversion processes to grow our private Independent Lives, homecare offer

Implementing robust KPI frameworks tailored to domiciliary care performance, quality and compliance

Ensuring safe, high‑quality homecare delivery aligned with CQC expectations and best practice

Providing visible, values‑led leadership during a period of significant change

Key Responsibilities
Lead Homecare Service Transformation & Performance
-
Deliver a 12‑month transformation programme with clear milestones and outcomes
-
Implement KPI frameworks covering scheduling, continuity of care, visit punctuality, capacity planning and quality

Strengthen reporting processes to ensure clear oversight and accountability

Improve efficiency, customer pathways and the overall homecare experience
Drive Growth & Sustainability

Increase private pay customers, hours and income through a strengthened homecare offer

Develop clear, consistent customer journeys from first enquiry to ongoing support
-
Contribute to financial planning, performance monitoring and long‑term sustainability
Provide Strong Leadership & Engagement

Build effective relationships with homecare teams, corporate functions and external partners
-
Support teams to understand roles, expectations and priorities following change

Communicate a clear vision for a modern, high‑quality homecare service

Foster a positive, inclusive culture focused on continuous improvement
Strengthen Quality & Governance

Ensure compliance with CQC, safeguarding, GDPR and health & safety requirements

Embed consistent, high‑quality standards and operational processes across all homecare teams

Identify risks early and take appropriate action to address performance issues
✅ What You’ll Need

Significant experience leading transformation or turnaround activity within a regulated independent homecare or care service

Strong understanding of domiciliary care operations, scheduling, workforce planning and quality governance
-
Proven ability to implement change and drive measurable service improvement

Experience developing KPI frameworks and using data to inform decision‑making
-
Excellent stakeholder management and communication skills
-
Commercial awareness, including understanding of growth and sustainability within independent homecare
-
A resilient, values‑led leadership style with the ability to bring teams through change

Required Criteria

Proven experience as a Transformation Manager or in a similar senior change leadership role.

Demonstrable experience in leading structured transformation programs within operational settings.

Experience in developing and modernising homecare operations or similar community-based care services.

Knowledge and experience in embedding clear team structures, accountability, and consistent ways of working.

Experience in improving enquiry handling, assessment pathways, and conversion processes to grow service offerings.

Ability to implement robust KPI frameworks tailored to domiciliary care performance, quality, and compliance.

In-depth understanding of CQC expectations and best practices for safe, high-quality homecare delivery.

Strong, values-led leadership skills, particularly in managing significant organizational change.

Ability to travel regularly across homecare teams and localities within Suffolk.

Availability for a 12-month fixed-term contract.
