Do you have a passion for supporting older people to live well, stay independent, and feel valued? We are looking for a Social Care Coordinator / Social Work Assistant to join our dedicated team and make a real difference in the lives of older adults, their families, and carers
As a Social Care Coordinator, you will work closely with social workers to deliver person-centred care and support. You’ll be the friendly, trusted link between older people, their families, and community services—helping to reduce isolation, build confidence, and promote wellbeing.
This role is perfect for someone who is compassionate, organised, and motivated to champion the rights and dignity of older people.
As a Social Care Coordinator / Social Work Assistant you will be:
- Supporting social workers in delivering tailored care plans.
- Carrying out activities that promote independence, resilience, and community connections.
- Visiting service users at home and liaising with families, carers, and health professionals.
- Building strong links with voluntary and community services.
- Monitoring and reviewing care packages to ensure they meet individual needs.
- Advocating for older people and promoting their rights and wellbeing.
You will have:
- Qualifications: 5 GCSEs (including English) OR NVQ Level 3 in Care (or equivalent).
- Experience: At least 2 years’ paid or voluntary experience supporting older people or those with disabilities in a care or hospital setting.
- Registration: Northern Ireland Social Care Council (NISCC) registration.
- Driving Licence: Full UK licence and access to a car (reasonable adjustments considered).
This role is subject to a clear ACCESSNI Enhanced Vetting Check to ensure the applicant can work in REGULATED ACTIVITY. A criminal record will not necessarily be a bar to an applicant obtaining a position
