New Life Fostering is an independent fostering agency dedicated to providing safe, nurturing homes where children and young people can thrive. As our service continues to grow, we are seeking an enthusiastic and committed Children and Young People Support Lead to help deliver outstanding support and positive outcomes for the children in our care.
About the Role
We are seeking a dedicated, organised, and child-focused professional to join our fostering team. This rewarding role combines direct support for children and young people with responsibility for monitoring educational outcomes, health needs, care planning, and engagement activities across our fostering service.
The successful candidate will play a vital role in ensuring children and young people receive the highest quality support, while working closely with foster carers, social workers, education providers, health professionals, and other key stakeholders.
Key Responsibilities
· Supporting Children and Young People.
· Provide one-to-one support for children and young people.
· Conduct bi-monthly visits with children in placement.
· Complete exit interviews with children and young people leaving placements.
· Gather feedback from looked-after children and birth children to support foster carer annual reviews.
Education Lead Responsibilities
· Monitor and track Personal Education Plans (PEPs), ensuring all are up to date.
· Maintain and update education tracking systems monthly.
· Monitor educational progress, including SATs, GCSE, and A-Level outcomes.
· Celebrate and recognise achievements by maintaining positive engagement with young people.
Health Lead Responsibilities
· Monitor and maintain records relating to children's health needs.
· Ensure children are registered with relevant health services.
· Track medical appointments, Looked After Child (LAC) medicals, prescribed medication, illnesses, and allergies.
· Maintain accurate and up-to-date health records.
Care Planning and Compliance
· Support the completion and monitoring of Care Plans and Pathway Plans.
· Monitor Training, Support and Development Standards (TSDS) submissions and ensure completion prior to panel reviews.
· Maintain accurate records and reports in line with organisational requirements.
Activities and Participation
· Plan, organise, and facilitate activities and events throughout the year.
· Coordinate holiday programmes, summer events, and end-of-year celebrations.
· Support and deliver the Next Steps Programme.
· Organise and coordinate monthly foster carer support groups.
· Facilitate the quarterly "Have Your Say" participation group for children and young people.
Partnership Working
· Attend professional meetings and reviews.
· Liaise effectively with Local Authority Social Workers, education professionals, health services, and other partner agencies.
· Work collaboratively to achieve positive outcomes for children and young people.
About You
You will be:
· Passionate about improving outcomes for children and young people.
· Highly organised with excellent administrative and record-keeping skills.
· Confident in building positive relationships with children, carers, and professionals.
· Able to manage multiple priorities and work independently.
· Flexible and willing to work evenings and weekends to support activities and events.
Essential Requirements
· Experience working with children and young people within fostering, social care, education, or a related field.
· Excellent communication and interpersonal skills.
· Strong organisational and planning abilities.
· Ability to maintain accurate records and manage confidential information.
· Need to be able to use excel to complete health and education data.
· Full UK driving licence and access to a vehicle.
