About the Role
Jewel Home Support is looking for a proactive and organised Care Coordinator to join our growing team based in Chorley. This is a rewarding role where you'll play a key part in ensuring our service users receive safe, responsive, and person-centred care by coordinating care packages, managing staff rotas, and supporting our care teams. This position also requires participation in our on-call rota, providing out-of-hours support during evenings and weekends on a shared basis.
Key Responsibilities
- Coordinating care packages and scheduling care staff.
- Managing daily rotas to ensure all visits are covered.
- Responding quickly to changes, staff shortages, and emergencies.
- Liaising with service users, families, healthcare professionals, and care staff.
- Supporting hospital discharges and new care package starts.
- Monitoring service quality and resolving day-to-day operational issues.
- Maintaining accurate records and ensuring compliance with company procedures.
- Participating in the shared on-call rota, including evenings and weekends.
