We are seeking an enthusiastic and experienced Children's Complex Case Manager to join our Children's Continuing Care Team. This is an exciting opportunity to play a pivotal role in ensuring that children and young people with complex health needs receive safe, effective and personalised continuing care services.
As part of a specialist team, you will work closely with children, young people, families and partner organisations to coordinate high-quality care and support positive outcomes. This role offers the opportunity to utilise and further develop your expertise in continuing care, complex case management and multidisciplinary working across North West London.
The Children's Complex Case Manager is the lead nurse assessor for children's and young people's continuing care and will be responsible for:
Leading comprehensive continuing care assessments, reviews and reassessments for children and young people with complex health needs.
Providing expert case management to ensure care is safe, effective, personalised and responsive to changing needs.
Working closely with families, carers, advocates and service providers to coordinate and oversee high-quality packages of care.
Sourcing and developing bespoke care solutions that meet individual needs and promote the best possible outcomes.
Collaborating effectively with health, social care, education and provider organisations across North West London.
Chairing and contributing to complex multidisciplinary meetings to support robust decision-making and care planning.
Supporting the delivery of excellent clinical outcomes and ensuring services are provided in line with national Continuing Care guidance and local policies.
