We’re excited to offer an opportunity for an experienced Primary Care Pharmacist and Independent Prescriber to join our dynamic and supportive practice team on a full-time basis. This is your chance to make a real impact on patient care from day one — with immediate start available (subject to standard checks).
This is a patient-facing, in-practice position at the heart of a collaborative healthcare environment. You’ll play a key role in optimising medicines use, improving patient outcomes, and supporting clinical decision-making across the practice and wider network. Experience in dealing with minor illness would be an advantage.
Your daily expected workload will include -
- Respond to medicines-related queries from GPs, PCN colleagues, practice staff, community pharmacies, and patients — providing clear, evidence-based guidance and solutions.
- Conduct clinical medication reviews for patients with complex needs, including multi-morbidity and polypharmacy.
- As an Independent Prescriber, make and implement prescribing decisions, request monitoring tests, and ensure safe and effective ongoing treatment.
- Review the continued need for medications, ensure appropriate monitoring, and empower patients to get the very best from their treatment plans.
- Work closely with colleagues across the practice, PCN, ICB, and external partners to deliver seamless care and build strong, lasting professional relationships.

Pay: £40,000.00-£55,000.00 per year
