Minimum Commitment: 15 hours per week
Working Pattern:
- Weekday shifts between 18:00 and 08:00
- Weekend and Bank Holiday cover from Friday 18:00 through to Monday 06:00, providing 24-hour out-of-hours clinical assessment services.
- Flexible shifts available within these operating hours.

Join Our Integrated Urgent Care Team
Partnering Health Group is a dynamic and growing healthcare organisation committed to delivering high-quality urgent and primary care services across Hampshire. We are passionate about providing safe, patient-centred care and are seeking experienced Advanced Nurse Practitioners (ANPs) and Advanced Clinical Practitioners (ACPs) to join our expanding Integrated Urgent Care Clinical Assessment Service (IUC CAS).
This is a fully remote working opportunity, allowing you to provide expert telephone and digital clinical assessments from home while being supported by an experienced multidisciplinary team. You will play a key role in delivering high-quality out-of-hours care, making autonomous clinical decisions and ensuring patients receive the right care, at the right time, in the right place.
The Role
As an Advanced Nurse/Clinical Practitioner, you will independently assess, diagnose, treat and safely manage patients presenting with urgent healthcare needs across all age groups. Working within the NHS 111 Integrated Urgent Care Clinical Assessment Service, you will provide remote clinical consultations and determine the most appropriate care pathway for each patient.
Key Responsibilities
- Provide autonomous remote clinical assessments via telephone and digital consultation.
- Assess patients of all ages presenting with acute, chronic and non-acute conditions.
- Take comprehensive clinical histories and formulate differential diagnoses and treatment plans.
- Prescribe safely and appropriately within your scope of practice.
- Refer patients to appropriate primary, community, secondary or emergency care services.
- Recognise deteriorating patients and initiate emergency interventions where required.
- Identify safeguarding concerns and take appropriate action in line with local and national guidance.
- Ensure patients receive clear safety-netting advice and appropriate follow-up arrangements.
- Deliver high-quality, person-centred care while meeting service performance standards.
- Maintain accurate, timely and comprehensive clinical records.
- Work collaboratively with the wider multidisciplinary team to deliver safe, effective patient care.
Essential Requirements
- Current registration with the NMC or HCPC.
- MSc Advanced Clinical Practice (or equivalent), including an Independent/Non-Medical Prescribing qualification.
- Minimum of 6 years post-registration experience, including:
- At least 3 years in General Practice, or
- 3 years within an Urgent Treatment Centre, Emergency Department or Integrated Urgent Care setting.
- Level 3 Adult and Children Safeguarding.
- Up-to-date mandatory training.
- Eligible to work in the UK.
- Enhanced DBS clearance.
- Demonstrable experience of autonomous practice across the four pillars of Advanced Clinical Practice.
- Excellent clinical assessment, communication and decision-making skills.
Desirable Skills
- Teaching or mentorship qualification.
- Experience using Adastra, EMIS, SystmOne and Electronic Prescription Service (EPS).
- Strong organisational and workload management skills.
- Experience within NHS 111 or an Integrated Urgent Care Clinical Assessment Service.
