Overall purpose of the role
The purpose of the Care Manager is to coordinate, plan and oversee person centred care for service users, ensuring their safety, health and quality of life.
You will act as a central point of contact between service users, families, external agencies and staff.
You will manage care planning, assessment and audits, some daily operations and staff.
You will receive a comprehensive induction and shadow training with another care manager, supported by the registered manager. There will also be further training opportunities.
Main responsibilities
- To support the Registered Manager in the delivery of service user care ensuring all key milestones for both service users and support workers are delivered on time
- Ensure safeguarding, robust risk management, and quality service delivery
- Initiate care plans and manage updates, ensuring all service users have current person-centred plans
- Support the registered manager with recruitment and interview processes for support staff
- Support the registered manager with support worker supervisions
- Organise and chair service user reviews, involving families, advocates, and professionals.
- Support the registered manager with new service user enquiries: conduct show-rounds, lead initial meetings, and coordinate trial stays.
- To undertake service users’ assessments, in the absence of the R.M.
- To ensure compliance to all CQC and LA regulatory requirements through robust management
- To ensure robust communication with all key professionals associated with the care delivery within placements
- To monitor, review and reassess individual packages of care depending on needs
- Ensure safety and quality
- To ensure robust completion of incident reporting and input onto local spreadsheets
- To ensure all service user and support worker contact is captured as required, highlighting those clients that require a high level of support and care to the RM
- To lead and manage the Team Leaders and support workers to ensure they can perform their roles safely and to the best of their ability
- To support and participate in the on-call function which will require some weekend work
- To support the audit and spot check process which will require some working of unsociable hours
The job description is not intended to be exhaustive, and it is likely that duties may be altered from time to time in the light of changing circumstances and after consultation with the post holder.
All staff are expected to actively participate in annual appraisals and set objectives in conjunction with their manager. Performance will be monitored against set objectives.
Confidentiality
All staff working for Vogue have a common law duty and a statutory duty of confidentiality to protect service user (and indeed any personally identifiable) information and only use it for the purposes for which it was intended. The disclosure and use of confidential service user information need to be both lawful and ethical.
Information Governance
All staff must keep up-to-date with the requirements of information governance and must follow Vogues policies and procedures to ensure that information is dealt with legally, securely, efficiently and effectively. Staff must appropriately manage the records they create or hold during the course of their employment with the Trust, making the records available for sharing in and confidentiality policies, procedures and guidelines (e.g. Freedom of Information Act 2000, Caldecott guidelines).
Safeguarding
Vogue believe safeguarding is everybody’s business. Abuse causes devastating consequences and damages survivors, throughout the rest of their lives. All employees therefore have a duty to safeguard and promote the welfare of our service users who are at risk. It is an essential requirement that staff are aware of the local Safeguarding procedures for sharing information about the welfare of any person for whom they have safeguarding concerns. Staff have a duty to ensure they attend training to enable them to recognise the indicators for concerning behaviour and receive safeguarding supervision as appropriate.
Health & Safety
All staff have a duty to ensure the health and safety of themselves and others whilst at work. Safe working practices and health and safety precautions are a legal requirement. ALL accidents must be reported to the registered manager and you must participate in accident prevention by reporting hazards and following relevant policies and procedures including Moving and Handling guidelines.
