We are looking for a MHSOP Social Worker to join our Adult Care Service, supporting older people and adults with a physical disability.
WHAT IS INVOLVED?
Based within the Dales Locality, you will work within a multi-disciplinary team to complete assessments of need and care plan for older people and adults with a physical disability.
You will ensure that services are delivered, monitored, and reviewed in accordance with the agreed care plan.

WHAT WILL I NEED?
To be successful in this role, you will need experience of working within Adult Social Care together with an up to date knowledge of multi-agency working and the associated adult care agenda.
You will need a Degree in Social Work or equivalent (eg. Dip SW, CSS or CQSW) plus Social Work England registration.

HYBRID WORKING
Durham County Council currently operates a hybrid working policy where a model of 2 days in the workplace and 3 days at home/remote will apply. This model is pro-rated for part time roles and arrangements are at managers discretion, alternatively you can work all of your contracted hours within the workplace if you prefer.
