We are seeking enthusiastic, compassionate and motivated UK-based Assistant Practitioners to join our dynamic Older People and Physical Disability (OP/PD) Team within Adult Social Care.

This is an exciting opportunity to work alongside experienced Social Workers, Occupational Therapists and Assistant Practitioners within a forward-thinking service that is embracing innovative and strengths-based ways of working.

About the Role

As an Assistant Practitioner, you will play a key role in supporting adults with physical disabilities and older people to maintain their independence, improve their wellbeing, and achieve their personal outcomes.

You will undertake assessments, reviews and support planning, identify preventative and community-based solutions, and work closely with individuals, carers, families and partner agencies to ensure people receive the right support at the right time.

The role provides an excellent opportunity to build professional knowledge and experience whilst making a meaningful difference to people’s lives.

Our service is actively developing approaches such as:

Community Social Work
Human Learning Systems
Community Led Support (CLS)

These approaches focus on early intervention, prevention, collaboration and creativity, helping people to draw upon their personal strengths, support networks and community resources before reliance on formal services becomes necessary.

About the Team

You will be joining a highly experienced, welcoming and supportive Older People and Physical Disability Team, committed to delivering outstanding person-centred support.

Our team supports adults who may be experiencing a wide range of complex circumstances, including physical disabilities, long-term health conditions, frailty, social isolation, self-neglect, safeguarding concerns and changing care and support needs.

We are proud of our positive team culture, where learning, development and collaboration are actively encouraged. Practitioners support one another through regular supervision, peer learning and shared problem-solving, ensuring that no one feels alone when managing complex situations.

We are looking for individuals who are:

Skilled communicators who can build effective and trusting relationships.
Creative, curious and solution-focused in their approach.
Organised and able to manage competing priorities effectively.
Confident using IT systems and electronic recording systems.
