An exciting opportunity for a highly skilled and motivated individual has arisen at the George Eliot Hospital NHS Trust to join the Acute Medicine Department as a Consultant Physician.

The successful applicant will join the Acute Medicine service comprising the Acute Medical Unit (AMU), Same Day Emergency Care (SDEC), and Hospital at Home (George Eliot @ Home) Service.

Acute Medicine is being reconfigured with the aim of offering 7 day service in order that our patients are provided with high quality clinical care at the front door and beyond, in partnership with our Accident and Emergency Department team.

Applicants should see this opening as an opportunity to be part of the George Eliot Hospital NHS Trust’s drive to innovate for the development of improved services for the benefit of our local community.

Provide consultant leadership and senior clinical decision-making across the Acute Medicine Service.

Deliver consultant-led ward rounds, multidisciplinary board rounds and timely review of patients within the Acute Medical Unit (AMU), Same Day Emergency Care (SDEC), and Hospital at Home (George Eliot @ Home).

Support the development of SDEC, Hospital at Home, and ambulatory pathways to improve patient outcomes and reduce unnecessary admissions.

Work collaboratively with the Emergency Department, Frailty Assessment Unit, Ambulatory Care Unit, Gastro Day Unit, specialty teams and community services to deliver integrated patient care.

Participate in the General Internal Medicine (GIM) on-call rota and the Acute Medicine weekend rota.

Lead and contribute to service improvement, quality improvement, clinical governance and audit activities.

Supervise, teach and support resident doctors and the wider multidisciplinary team.

Play an active role in shaping the future development of Acute Medicine and innovative models of urgent and emergency care at George Eliot Hospital.

Provide consultant-led clinical care and leadership across the Acute Medicine Service, including the Acute Medical Unit (AMU) Same Day Emergency Care (SDEC), and Hospital at Home (George Eliot @ Home)

Deliver timely senior clinical assessment and decision-making to support early diagnosis, treatment, admission avoidance and safe discharge.

Undertake consultant ward rounds, multidisciplinary board rounds and review of patients within Acute Medicine and SDEC.

Support the development of Same Day Emergency Care (SDEC) pathways and work collaboratively with George Eliot @ Home service to identify patients suitable for admission avoidance and early supported discharge.

Work closely with the Emergency Department, Frailty Assessment Unit, Planned Ambulatory Care, Gastro Day Unit, inpatient specialties and community teams to provide integrated, patient-centred care.

Contribute to the ongoing development of Acute Medicine services, ensuring pathways reflect national best practice and local service priorities.

Participate in the Acute Medicine and General Internal Medicine (GIM) on-call rota.

Participate in the development and implementation of clinical guidelines, audit, quality improvement initiatives and all aspects of the clinical governance agenda.

Demonstrate flexibility in undertaking appropriate clinical duties as requested by the Clinical Lead to meet service requirements.

Actively contribute to undergraduate and postgraduate education, providing supervision, teaching and support for doctors in training, advanced clinical practitioners and the wider multidisciplinary team.

Maintain and, where appropriate, develop a specialist clinical interest that complements the needs of the Trust and Acute Medicine Service.

Develop and maintain effective working relationships with General Practitioners, community providers and external partner organisations.

Comply with all Trust policies, procedures and statutory requirements, including Health and Safety, Information Governance, Data Protection, Incident Reporting and Standing Financial Instructions.

Maintain high standards of clinical practice through participation in Clinical Governance, Risk Management, audit and service evaluation.

Take responsibility for the educational supervision, appraisal and professional development of trainee doctors in accordance with national guidance.

Maintain Continuing Professional Development (CPD), participate in annual appraisal and support the Trust's revalidation process in line with Royal College of Physicians requirements.
