We are looking for an experienced, organised and compassionate Care Coordinator to join our growing team in Renfrew.

This is an important role within our operational team. You will play a key part in ensuring our service users receive safe, consistent and outcome-focused care, while providing leadership, guidance and support to our frontline care team.

About the Role

As Care Coordinator, you will coordinate and oversee care delivery across an allocated service area, ensuring that services are delivered in line with assessed needs, care plans, commissioned hours, contractual requirements and CRC Care's values.

You will work closely with service users and their families, care staff, Social Work teams, health professionals, commissioners and other external partners.

Your responsibilities will include:

Coordinating day-to-day care delivery across your allocated service area.
Maintaining safe, effective and appropriately staffed rotas.
Providing leadership, supervision and support to frontline care staff.
Carrying out care package and care plan reviews, normally every six months or sooner where required.
Monitoring care quality, outcomes and compliance with agreed care plans.
Supporting medication governance and undertaking six-monthly medication audits where applicable.
Monitoring missed visits, late attendance, alerts and concerns through our digital homecare management systems and taking appropriate action.
Recording, investigating and responding to complaints in accordance with organisational procedures.
Ensuring accidents, incidents, notifiable events and Adult Support & Protection concerns are appropriately reported and escalated.
Supporting recruitment, selection, induction and onboarding of new staff.
Ensuring mandatory training, shadowing and competency requirements are completed.
Conducting regular staff supervisions and annual appraisals within your area of responsibility.
Identifying staff development and training needs.
Maintaining accurate electronic records and service user documentation.
Providing clear handovers and weekly operational reports to the Registered Service Manager.
Liaising with Social Work, District Nursing, GPs, Occupational Therapy, Community Mental Health teams, HSCPs and Local Authority commissioning and monitoring teams.
Supporting service users and families and responding promptly to concerns.
Supporting Care Inspectorate inspections, local authority monitoring visits, audits and quality assurance activity.
Contributing to service improvement and identifying opportunities to improve the way we deliver care.

What We're Looking For

We are looking for someone who can demonstrate strong leadership, organisation and communication skills and who understands what good-quality care looks like.

Essential

SVQ Social Services & Healthcare Level 3 (SCQF Level 7), or currently working towards this.
Registered with, or eligible to register with, the Scottish Social Services Council (SSSC) as a Supervisor.
Experience supervising frontline care staff.
Experience coordinating staff rotas and care delivery.
Excellent organisational and communication skills.
Strong IT skills and confidence using Microsoft Office.
Experience of, or familiarity with, digital homecare management systems such as CareLineLive, LogMyCare, Birdie or similar.
Excellent attention to detail and the ability to manage competing priorities.
Knowledge of Adult Support & Protection and the Adult Support and Protection (Scotland) Act 2007.
Full UK driving licence and access to a vehicle.
Successful membership of the Protecting Vulnerable Groups (PVG) Scheme.

Desirable

SVQ Social Services & Healthcare Level 4 (SCQF Level 9) or another relevant leadership/supervisory qualification.
Previous experience working within local authority commissioned home care services.
