Social Care Officer - MIST Team (Ref: ASC/26/044/LLS)

Hours: 37 per week

Location: Medway

Working style:

Hybrid – Works part from Medwat and part from home.

We are excited to announce a new opportunity for a Social Care Officer to join our Newly established Medway’s Intensive Support Team (MIST) within Adult Social Care. This role presents a unique chance to make a significant impact on the lives of individuals facing multi-faceted complexities, providing hands-on support and interventions within the community including but not exclusive to acute settings, individual’s homes and numerous other settings. The successful applicant will be required to have experience within Mental Health, Learning Disabilities, alcohol and Drug use, Safeguarding, homelessness, and outreach work.

As a Social Care officer in this dynamic team, you will utilize yourself as a tool to engage directly with clients, often in crisis situations, and those that require a multi-disciplinary team (MDT) response. Your role will involve advocating for individuals, ensuring their needs are met, and facilitating access to vital services. Strong communication skills are essential, as you will be required to build and maintain relationships with individuals accessing support, their families, and partner agencies, including housing, police, probation services, mental health professionals, and addiction support organizations.

Knowledge of the Medway area is crucial, as you will be navigating local resources and voluntary services to enhance the support available to individuals. Whist in Medway we support agile working, given the nature of this work, it will predominantly require a presence in both office and community settings, emphasizing direct engagement with clients to effectively address their needs.

You will need to apply a strength-based approach in your practice, supporting/undertaking Care Act assessments and devising care plans through our three-conversation framework of assessment. An understanding of safeguarding principles, the Mental Health Act, and the Mental Capacity Act is fundamental to ensure the safety and wellbeing of those you support.

Career Progression Frameworks (CPF) have been introduced for all roles at Medway Council. These frameworks provide a mechanism for employees to take ownership of their development within their post and be rewarded accordingly. This means that the salary you will be offered will be at level A unless, based on an assessment against the CPF criteria, you demonstrate that you are meeting the criteria of level B or C. There will be further opportunity once in post to progress from level A to B, and B to C.

Level C requires the highest level of competence and applicants would need to fully meet and demonstrate all aspects of the job profile, including qualifications, knowledge, skills and experience.

Level A

Medway Intensive Support Team are seeking 3 x Social Care Officers to join the team at level A. This is the entry point of the role, so you will need to be able to demonstrate the relevant knowledge, experience and skills to get you started, but you will be supported to develop in your role through induction, probation and a supportive Performance Appraisal and Career Conversation. All elements of level A listed within the person specification are essential. Through your induction you will be introduced to the Career Progression Framework (CPF), which will help you to identify your areas of strength and any development needs.
