Salary: £26,500 - £28,000 per annum (depending on experience)

Full-Time | Participation in On-Call Rota Required

We are looking for an organised, compassionate, and proactive Care Coordinator to join our dedicated office team. This is a fantastic opportunity for someone who has experience in Care Cordinating role within a care setting.

About the Role

As a Care Coordinator, you will play a vital role in ensuring our clients receive the highest standards of care and continuity of service. Working closely with the Registered Manager, care team, clients, families, and healthcare professionals, you will coordinate care delivery, manage schedules, and provide ongoing support to both clients and Care Professionals.

Key Responsibilities

Create and manage client and Care Professional rotas to ensure safe and effective care delivery.
Coordinate care packages, Live in and Day centre Packages and respond to changes in client needs.
Liaise with clients, families, healthcare professionals, and the care team.
Use care management software to manage schedules, generate reports, invoices, and payroll information.
Maintain accurate and compliant client and staff records.
Handle new care and recruitment enquiries professionally.
Support recruitment, onboarding, supervision, and training of Care Professionals.
Participate in the on-call rota, providing support outside office hours.
Assist with quality assurance, compliance, safeguarding, and CQC requirements.
Support the Registered Manager and provide cover during periods of absence.
Ensure exceptional customer service and promote Radfield Home Care's values at all times.

About You

We're looking for someone who is:

Experienced in domiciliary care coordination or a similar role.
Knowledgeable about care planning systems and rota management.
Passionate about delivering outstanding person-centred care.
Highly organised with excellent administrative skills.
Confident using IT systems and managing multiple priorities.
An excellent communicator with strong relationship-building skills.
Able to work independently and use initiative.
Committed to maintaining confidentiality and professional standards.
Flexible and willing to participate in an on-call rota.

Essential Requirements

Experience working within a domiciliary care setting.
Experience creating and managing care rotas.
Full UK driving licence and access to a vehicle.
Excellent literacy, numeracy, and administrative skills.
Understanding of safeguarding and care sector compliance.
Enhanced DBS check (or willingness to obtain one).
