About the Role

This is a central role in our team and a great opportunity for someone who enjoys ensuring the efficient delivery of high-quality domiciliary care services through our wonderful team of carers. The post holder will be responsible for managing staff rotas, scheduling care visits, and, aspart of a wider team, maintaining effective communication with clients, families, care workers, and healthcare professionals.

The Care Co-ordinator will ensure that care services are delivered safely, consistently, and in accordance with client care plans, company policies, and regulatory requirements.

What You’ll Be Doing

Care Coordination

Coordinate and schedule care visits to ensure clients receive support in line with their agreed care plans.
Work with the Registered Manager to match care workers to clients based on skills, experience, availability, location, and client preferences.
Respond promptly to changes in client needs or circumstances.
Liaise with clients, family members, healthcare professionals, local authorities, and other stakeholders regarding care arrangements.
Support the assessment and review process by providing operational information and feedback.

Rota and Workforce Management

Create, maintain, and manage daily, weekly, and monthly staff rotas.
Ensure all care visits are allocated and covered appropriately.
Work with the wider office team to optimise staff availability, considering annual leave, sickness, training commitments, and other absences.
Arrange emergency cover and make adjustments to schedules at short notice when required.
Monitor travel times and route efficiency to maximise productivity while maintaining quality care.
Ensure staffing levels meet operational requirements and contractual obligations.
Assist with workforce planning and capacity management.

Staff Support and Communication

Act as a primary point of contact for care workers regarding schedules, client updates, and operational issues.
Maintain regular communication with care staff to ensure effective service delivery.
Support the induction and integration of new care workers into service schedules.
Promote a positive team culture and provide day-to-day operational support.

Quality and Compliance

Ensure care delivery complies with company policies, safeguarding procedures, and relevant legislation.
Maintain accurate and up-to-date records using electronic care management systems.
Monitor missed calls, late visits, and service exceptions, taking corrective action where necessary.
Support audits, inspections, and quality assurance activities.
Ensure confidentiality and compliance with data protection requirements.
Report incidents, accidents, complaints, and safeguarding concerns promptly and appropriately.

Customer Service

Build positive professional relationships with clients and their families.
Handle enquiries and concerns in a professional and timely manner.
Promote person-centred care and ensure clients' dignity, independence, and preferences are respected.
Contribute to maintaining high levels of client satisfaction and retention.

Person Specification

Essential CriteriaExperience

Previous experience within a domiciliary care, health, or social care environment.
Experience of staff scheduling, rota management, or care coordination.
Experience working in a fast-paced environment with competing priorities.
Experience communicating with clients, families, and multidisciplinary professionals.

Knowledge

Understanding of domiciliary care services and person-centred care principles.
Knowledge of safeguarding adults and children.
Understanding of care quality standards and regulatory requirements.
Knowledge of workforce planning and scheduling practices.

Skills

Excellent organisational and time-management skills.
Strong communication and interpersonal abilities.
Ability to work under pressure and respond effectively to changing priorities.
Problem-solving and decision-making skills.
Competent IT skills, including care management and rostering software.
High level of attention to detail and accuracy.

Desirable Criteria

NVQ/QCF Level 3 or above in Health and Social Care.
Experience using electronic rostering and care management systems.
Experience working within a regulated care service.
Knowledge of local authority commissioning and care package management.

Pay: £27,300.00 per year
