Full job description

At Quality Care (SC), being a Care Coordinator is about much more than managing rotas. You'll play a key role in delivering high-quality, person-centred care by coordinating care services, supporting our care team, and ensuring our clients receive the highest standard of care.

This is a varied role that combines office-based coordination with field-based responsibilities, including conducting assessments, care reviews, spot checks, and supporting care staff. As the role requires regular travel within the community, you must hold a full UK driving licence and have access to your own vehicle.

Key Responsibilities

Plan, organise, and coordinate weekly care rotas to ensure efficient service delivery.
Schedule care visits while considering travel times, continuity of care, and carers' work-life balance.
Ensure sufficient staffing levels to deliver care safely and on time.
Arrange cover for staff sickness, annual leave, and other absences.
Conduct client assessments, care plan reviews, and risk assessments.
Work alongside the Registered Manager to implement new care packages and match clients with suitable care staff.
Liaise with healthcare professionals, including social workers, GPs, and community nurses, to deliver person-centred care.
Respond professionally and compassionately to enquiries from clients and their families.
Carry out telephone monitoring, quality assurance checks, and service user visits.
Participate in the paid on-call rota and provide direct care when required.
Maintain accurate electronic and paper-based records in line with company policies and regulatory requirements.
Identify opportunities to improve services and support business growth.

Requirements

Minimum 1 year's experience as a Care Coordinator within domiciliary/home care.
SVQ Level 3 in Health & Social Care (preferred).
Full UK driving licence and access to your own vehicle.
Strong IT, organisational, and administrative skills.
Excellent communication and customer service skills.
Reliable, trustworthy, and able to work with integrity.
Self-motivated, proactive, and able to work independently using your own initiative.
Excellent attention to detail and ability to manage multiple priorities.

Please note: We are unable to offer visa sponsorship for this position.
Participation in the paid on-call rota is required.
