Overview
We are seeking a dedicated and compassionate Care Planner to join our team, specialising in senior care within a care home environment. The ideal candidate will be responsible for developing personalised care plans, coordinating services, and ensuring the highest standards of care are maintained. This role offers an opportunity to make a meaningful difference in the lives of the people we look after while working within a supportive and professional setting. The Care Planner will require excellent organisational and communication skills, along with experience in supervising and leading care teams. A valid driving licence is desirable to facilitate travel between care facilities or home visits as needed.

Responsibilities

Develop, review, and update personalised care plans tailored to individual resident needs, ensuring compliance with regulatory standards.
Coordinate with healthcare professionals, families, and care staff to deliver comprehensive and consistent care services.
Supervise and support care staff in implementing care plans effectively, fostering a positive team environment.
Monitor residents’ progress and adjust care strategies accordingly to optimise wellbeing and safety.
Maintain accurate documentation using IT systems, ensuring all records are up-to-date and compliant with data protection regulations.
Organise training sessions or meetings for staff regarding new procedures or updates to care protocols.
Facilitate effective communication between residents, families, and multidisciplinary teams to promote transparency and trust.
Assist in organising transport logistics for residents’ appointments or outings as required.

Experience

Proven experience working within a care home setting, particularly in senior or specialised eldercare roles.
Supervising or leadership experience within a healthcare or social care environment is highly desirable.
Strong knowledge of developing and managing care plans tailored to individual needs.
Excellent communication skills with the ability to liaise effectively with residents, families, and multidisciplinary teams.
Proficiency in using IT systems for documentation and reporting purposes.
Valid driving licence preferred to support mobility requirements across different locations or home visits. This role offers an engaging opportunity for a motivated professional committed to delivering exceptional senior care through effective planning and leadership.
