Overview
We are seeking a dedicated and experienced Complex Care Coordinator to oversee and coordinate personalised care plans within a senior care environment. The ideal candidate will possess strong leadership and supervising skills, with a focus on delivering exceptional care tailored to individual needs. Responsibilities
- Develop, implement, and review personalised care plans in collaboration with residents, families, and healthcare professionals
- Supervise and support care staff to ensure high standards of service delivery
- Coordinate daily activities and ensure smooth communication between team members and external agencies
- Monitor residents’ health and wellbeing, escalating concerns as necessary
- Organise training sessions for staff to maintain compliance with care standards and regulations
- Maintain accurate documentation of care activities and resident records using IT systems
- Lead on the organisation of appointments, medication management, and other essential services
- Foster positive relationships with residents, families, and external stakeholders to promote a supportive environment
Skills
- Proven experience in supervising or managing within a care home or senior care setting
- Strong leadership qualities with the ability to motivate and guide teams effectively
- Excellent communication skills, both written and verbal, to liaise confidently with residents, families, and healthcare professionals
- Competent in using IT systems for documentation, scheduling, and reporting purposes
- Valid driving licence is desirable for travel between care facilities or appointments
- Knowledge of developing and managing personalised care plans tailored to individual needs
- Ability to demonstrate empathy, patience, and professionalism in all interactions

Pay: £24,420.00-£42,880.77 per year
