Full job description

Overview
We are seeking a dedicated and skilled ANP or Paramedic Practitioner to join our clients Home visiting team on a long term locum contract. You will be working alongside a MDT covering visits mainly at various Care Homes and work closely with the Frailty/Care Home team. This work with be via a PCN so most of the visits / ward rounds will centre around Primary Care issues.

Responsibilities

Visits and ward rounds at Care Homes
Collaborate with other healthcare professionals to ensure comprehensive care for patients.
Maintain accurate patient records and documentation in compliance with legal and organisational standards.
Conduct thorough assessments of patients’ medical histories and current conditions.
Participate in ongoing training and professional development to stay current with best practices in emergency medical services.

Qualifications

Valid NMC / HCPC Pin
Previous Experience in Primary Care desirable but not essential
Independent Prescribing desirable but not essential
Proven experience in community patient care, ideally including visiting shifts
Excellent communication skills, both verbal and written, with the ability to work collaboratively within a team.
A valid driving licence , as travel is required

Job Types: Full-time, Part-time, Temporary, Fixed term contract
Contract length: 3 months

Pay: £35.00-£50.00 per hour
