Job summary
We are recruiting a Band 5 Occupational Therapist to join a newly established Community Rehabilitation Team (CRT) within the Mid and North Hampshire Division.
This is an excellent opportunity for a newly qualified or early-career OT to develop skills within a specialist community mental health rehabilitation service, supporting adults with complex needs to build independence, develop daily living skills and sustain recovery in the community.
You will hold a defined caseload and deliver person-centred, recovery-focused interventions, working alongside a supportive multidisciplinary team.
The focus of the service is on functional recovery, supporting people to develop the skills needed for independent living, including self-care, social participation and meaningful activity.
The team operates using an assertive outreach model, delivering flexible, community-based care within people's homes and local environments.
As a Band 5 Occupational Therapist, you will:
Provide occupational therapy assessment and intervention for individuals with complex needs
Support delivery of goal-based rehabilitation plans focused on independence
Deliver both individual and group interventions to improve occupational performance
Work closely with senior clinicians to develop skills in clinical reasoning and risk management
Contribute to multidisciplinary care planning and reviews
Support service users to access community resources, education and meaningful activity

Main duties, tasks & skills required
The post holder will:
Manage a caseload of service users with complex rehabilitation needs
Complete occupational therapy assessments, care planning and interventions in community settings
Deliver practical, skills-based interventions to support daily living (e.g. self-care, routines, budgeting, community access)
Build therapeutic relationships to engage individuals who may be unmotivated or difficult to engage
Develop and deliver group-based rehabilitation programmes
Contribute to risk assessment and safe, recovery-focused care
Work collaboratively within the multidisciplinary team and external partners
Maintain accurate clinical records and reports
Supervise Occupational Therapy assistant
Key skills required:
Ability to apply the Occupational Therapy process in mental health
Strong communication and engagement skills
Ability to work independently while seeking support appropriately
Good time management and prioritisation skills
Understanding of risk, safeguarding and recovery-based practice
